Administering Medicines

If your child requires any prescribed medication administered during the school day, the following form must be completed and handed to your child's class teacher along with the medication.

 

Please note. No medication can be administered without a completed and signed form. Please click below to download a copy of the form. Hard copies are also available from the office.
